Jack Howard Fields, 69, of Evarts departed this life on Friday, March 15, 2024, following a long illness. Born January 15, 1955 in Cincinnati, OH, he had lived here most of his life. Jack worked for himself as a contractor for over 20 years and was a member of the Kentucky National Guard.  He believed in the Holiness faith.

Jack is preceded in death by his parents, Woodrow Wilson Fields and Myrtle Price Fields; his beloved step-mother, Mary Fields Trett; a sister, Patsy Ruth Bennett; and two brothers, Mike Fields and Reverend Woody Fields.

Survivors include a brother, James Fields and wife Cathy, Evarts; three sisters, Linda VonWahhausen, Michigan; Janice Fields, Michigan; and Mary "Susie" Wilder, London, KY; special sister-in-law, Tammy Fields, Evarts; and a host of nieces, nephews, other family and friends including a special niece, Lori Patterson, Harlan.

Visitation will be held Sunday from 5 until 8 pm at the Evarts Funeral Home.

Funeral services will be conducted Monday, March 18, 2024, at 1 pm at the Evarts Free Pentecostal Holiness Church with Pastor Woody Fields and the Reverend Jarrod Fields officiating.

Burial will follow in the Evarts Memorial Gardens with family and friends serving as pallbearers.
